Estropipate Dosing for Low Estrogen in Younger Women

The recommended starting dose of estropipate for estrogen replacement in younger women who have low estrogen levels due to a variety of causes is 1.25 mg once daily. Your healthcare provider may increase your dose as necessary up to 7.5 mg once daily.
For this use, estropipate should be given cyclically (usually three weeks on, followed by eight to ten days off). This will help mimic the natural hormonal changes that occur with the menstrual cycle and will usually cause regular monthly periods.

A Note About Estropipate Strengths

Estropipate tablets come in the following strengths:
  • Estropipate 0.625 mg (which contains 0.75 mg estropipate per tablet)
  • Estropipate 1.25 mg (which contains 1.5 mg per estropipate per tablet)
  • Estropipate 2.5 mg (which contains 3 mg of estropipate per tablet).
The strengths of estropipate can be a little confusing. The "official" strength is calculated in terms of sodium estrone sulfate, even though the tablets contain slightly more estropipate (also known as piperazine estrone sulfate). You may find your pharmacy uses other ways to describe the strength. Just remember that estropipate (and its generic versions) only comes in three strengths. Estropipate 0.625 mg is exactly the same as estropipate 0.75 mg.

General Information on Your Estropipate Dosage

Considerations for women using estropipate include the following:
  • The medication comes in tablet form. It is taken by mouth once a day.
  • You can take estropipate with or without food. If the drug bothers your stomach, try taking it with food (some women find estrogens irritating to the stomach).
  • Estropipate is usually taken cyclically, with regular breaks to allow for period-like bleeding. Your healthcare provider will help you decide exactly how to take the drug.
  • It does not matter what time of day you take estropipate, although it is best to take it at the same time each day.
  • The tablets may be broken in half, if necessary, to achieve the proper dosage.
  • For the medication to work properly, it must be taken as prescribed.
